These are the default questions in the Exit Survey, sent after an employee leaves to learn why.

These standard questions are the same for every organization and can't be changed or removed. You can add your own questions alongside them. See What are supplemental questions?

Home-Based Care organizations get a slightly different version of this survey. Questions below are marked where they appear in only one version.

• How likely would you be to recommend employment there to someone else? (Very Unlikely to Very Likely)
• How would you rate your employer's ability to have properly matched you with a client? (Very Poor to Excellent) (Home-Based Care only)
• Please rate your level of agreement with these statements: (Strongly Disagree to Strongly Agree)
  • I had good friends at work. (Senior Living only)
  • I had a good relationship with my direct supervisor. (Senior Living only)
  • I received recognition and appreciation for my work.
  • My job allowed for good work/life balance.
• What would you say is the main reason other employees leave [your organization's name]?
• What would you say you liked or appreciated most about working there?
• Is there anything specific that you would recommend they improve?
