New Hire Survey Question Sets
These are the default questions in the New Hire Surveys, sent at 15, 30, 60, and 90 days to see how a new hire is settling in.
These standard questions are the same for every organization and can't be changed or removed. You can add your own questions alongside them. See What are supplemental questions?
The 60-day survey also has a Home-Based Care version. Its questions are marked below where they differ.
15-Day Survey

30-Day Survey

60-Day Survey

- Do you feel that you understand what's expected of you? (No or Yes)
- How are things with your supervisor? (Really Bad to Really Good) (Senior Living only)
- Outside of training, which coworkers have helped you feel comfortable in your new job? (Senior Living only)
- How would you rate your employer's ability to properly match you with a client? (Very Poor to Excellent) (Home-Based Care only)

90-Day Survey
